Photo Release Form

AUTHORIZATION TO USE PHOTOGRAPHS AND/OR AUDIO-VISUAL MATERIAL

The undersigned hereby authorizes Greater Naples Leadership, Inc., (“GNL”), a Florida not-for-profit corporation, to use, reproduce, exhibit and/or publish photographs and/or video recordings that may pertain to me, including my image, likeness and/or voice for internal use, all without compensation. This authorization includes the right to edit, mix or duplicate and use the material in whole or in part and in any format, now known or later developed, as GNL may elect, including GNL’s Internet Web site, restricted to member access and internal use only. I understand that the Material may be used in publications, brochures, press releases, social media postings, or for other promotional endeavors pertaining to GNL only with your permission.

This authorization is continuous and may only be withdrawn as to future use by my specific written rescission of this authorization delivered to GNL with receipt thereof acknowledged by its President or President-Elect.
